Service Access Liaison Program

The Family Resource Network, Inc. Service Access Liaison program (SAL) assists families in the Broome DDRO Region of NYS as they navigate the Office for People with Developmental Disabilities (OPWDD) system. Service Access with provide the following:

-Front Door eligibility guidance
-Post Assessment referalls to FSS programs
-Up to date list of professionals as a referral source for evaluations
-"Light Touch" approach to service coordination
-Information and Referral
-Training & information sessions on Service Access and OPWDD systems
-Advocacy
-Limited assistance in transportation for eligibility related issues

FRN's Service Access Liaison (SAL) program supports families living with and raising loved ones with intellectual and developmental disabilities through direct referrals, educational opportunities, outreach, and collaboration with state and voluntary agencies.

By providing a clearinghouse of information to community members, professionals, and individuals with disabilities and their families, SAL strives to assist families in obtaining services that will maintain their loved one's desired quality of life.

Service Access Liaison will serve individuals with disabilities in the following Broome DDRO counties: Broome, Chenango, Delaware, and Otsego.
